           Wilson walked by a patient whom he knew had recently had a hysterectomy walking down the hall that evening.
           "Ma'am, it's good that you're exercising, but where did you get the Dove bar?"
           "It fell off a truck!" she laughed.
           "A truck?"
           "Yeah, a truck with a broken axle!"
           Wilson never realized he'd been given a hint.

"it fell off a truck" is an American expression sometimes implying that something was acquired by rather sneaky means. The broken axle is a reference to House's leg being messed up, and there was also a subtle pun in-joke in the fact that the British English word for truck is "lorry".
